What Your Gut Does With Each One
The mechanism here is unusually well characterized for a supplement question.
Gut bacteria metabolize free choline into trimethylamine, which your liver then oxidizes into TMAO. A study published in the New England Journal of Medicine established that this conversion is microbiota-dependent, demonstrating it by suppressing TMAO with antibiotics and watching it return once the antibiotics stopped. The same work associated higher TMAO with major adverse cardiovascular events, independent of traditional risk factors.
Then the form question. A randomized, double-blind crossover trial in 37 healthy men gave 600 mg of choline as either choline bitartrate – a free form – or phosphatidylcholine – a bound form. The free form produced roughly three times the plasma TMAO. The bound form produced no more than the no-choline control.
And the part that makes this a gut health question rather than a chemistry one: the men who produced the most TMAO had measurably different bacterial populations, with more abundant Clostridium lineages from the Ruminococcaceae and Lachnospiraceae families. The same dose in two people produced different results depending on what was living in their intestines.
How Much Weight the Stroke Signal Carries
This deserves careful handling, because it is the strongest argument in the comparison and it is also observational.
A cohort study of more than 12 million Korean adults aged 50 and over, none with prior stroke, Alzheimer’s or cerebrovascular disease, found alpha-GPC users had a 46 percent higher ten-year incident stroke risk than matched non-users. The adjusted hazard ratio was 1.43 for total stroke, 1.34 for ischaemic and 1.37 for haemorrhagic. Longer use tracked with higher risk.
Now the caveats, which matter. This is a cohort study, not a trial, so it establishes association rather than causation. People prescribed alpha-GPC may differ from people who are not in ways the adjustment did not capture. And the TMAO mechanism, while plausible, was not measured in that study.
What makes it hard to dismiss is the dose-response pattern and the mechanistic story behind it. An association that strengthens with duration, in a population that large, with a plausible pathway, is not proof. It is also not nothing.
And then a second Korean study reached the opposite conclusion. Looking at 508,107 patients already diagnosed with mild cognitive impairment, it found alpha-GPC users had a lower risk of ischaemic stroke, hazard ratio 0.833, and of haemorrhagic stroke, 0.847, across the study population as a whole. That reduction held in patients whose impairment did not progress to dementia and disappeared in those whose did, where the association was not significant. Its authors concluded there is no reason to avoid alpha-GPC purely to prevent a stroke.
The two studies looked at different populations, which may explain much of the difference. The first examined adults over 50 with no history of cerebrovascular disease, while the second recruited patients who already had a cognitive diagnosis. There is also an important potential confounder: alpha-GPC is a prescription drug in South Korea, so patients receiving it were already being treated for cognitive complaints. Those complaints can themselves be an early marker of vascular disease, making it difficult to separate any effect of alpha-GPC from the reason it was prescribed in the first place.
So the honest position is that two large observational studies disagree, and neither settles it. That is precisely the situation this article’s own myths section describes, where a finding is treated as proof or as noise when it is neither.
